Membership Application Information
Please fill out a SEUEPA membership application form. You may return it via mail, fax, or as an attachment in an e-mail. If you do not send a membership check, SEUEPA will send an invoice in the amount indicated on the membership application. Payment by credit card is possible. Contact Sue Heino for more information.
